Given the nature of that webcomic, I think they start with old images from the Age of Engraving and then modify them in cunning ways, then add a witty caption. I suspect this of having been a “real” medical illustration to which they added the anchor. However, it’s possible that I’m completely misunderstanding their artistic process.
